Museum of Art Fort Collins (MoAFC)Youth Programs General Release Form – 2026

This release form is valid for all MoAFC youth programs from January 1, 2026 through December
31, 2026.

I hereby acknowledge and understand the nature of the activities provided through the Museum
of Art Fort Collins (MoAFC) youth programs, including but not limited to camps, classes,
workshops, and special events. By signing this waiver, I confirm that I have read, understood,
and agreed to the terms and conditions outlined below for the calendar year 2026:

1. Assumption of Risk: I am aware that participation in MoAFC youth programs involves
   certain inherent risks and hazards. These risks include, but are not limited to, accidents,
   injuries, and exposure to natural elements. I understand that the program organizers and
   staff will take reasonable precautions to minimize these risks, but some risks are
   unavoidable. I voluntarily accept and assume all such risks associated with the program
   activities for my child.
2. Medical Authorization: I authorize the program organizers, their designated staff, or
   emergency medical personnel to administer first aid and/or seek medical treatment for
   my child, if necessary, in the event of an injury, illness, or medical emergency during
   program activities. I understand that the program organizers will make reasonable efforts
   to contact me or the emergency contacts listed on the registration form in such
   situations.
3. Code of Conduct: I understand that my child is expected to follow the rules and
   guidelines set by the program organizers and staff. I agree to instruct my child to behave
   in a respectful and responsible manner towards fellow participants, staff, equipment,
   facilities, and the environment. I acknowledge that failure to comply with the code of
   conduct may result in disciplinary actions, up to and including dismissal from the
   program without refund.
4. Supervision and Liability: I acknowledge that the program organizers and staff will
   exercise reasonable care and supervision over program activities. However, I
   understand that they cannot be held liable for any loss, damage, injury, or accident that
   may occur during program activities, except in cases of gross negligence or willful
   misconduct.
5. Photography and Media Release: MoAFC may take photographs, videos, and other
   media recordings during program activities for promotional and educational purposes,
   including but not limited to program brochures, websites, social media, and news
   releases. By signing this form, I grant permission for my child’s image to be used in such
   materials. Children’s full names will not be used in publicly shared materials without explicit parent consent.
6. Transportation: I understand that some programs may include walking field trips to
   nearby locations or outdoor activities on museum grounds (Oak Street Plaza.) I give
   permission for my child to participate in these activities under staff supervision. For any
   programs requiring vehicle transportation, separate permission will be requested.
7. Snack and Food Policy: I understand that snacks may be provided during some
   program activities. If I do not provide lunch or snacks for full-day programs, I hereby
   grant permission for MoAFC to provide snacks for my child, keeping any food allergies
   listed on the registration form in mind. I will notify staff immediately of any changes to my
   child’s dietary restrictions or allergies.
8. Personal Belongings: I understand that MoAFC is not responsible for lost, stolen, or
   damaged personal belongings brought to program activities. I will ensure my child brings
   only necessary items and will label them clearly.
9. Program Changes: I understand that program schedules, activities, and staff
   assignments may be subject to change due to circumstances beyond MoAFC’s control.
   MoAFC will make reasonable efforts to notify families of significant changes.
10. Acknowledgment of Information Accuracy: I certify that all information provided on
    the registration and medical information forms is accurate and complete. I agree to notify
    MoAFC immediately of any changes to contact information, medical conditions, allergies,
    or emergency contacts during the 2026 program year.
11. Right to Modify or Revoke Permissions: I understand that I may request changes to
    specific permissions granted in this release form (such as photography consent) at any
    time by submitting a written request to MoAFC staff. I also understand that I have the
    right to revoke this release form entirely, which will result in my child’s withdrawal from all
    MoAFC youth programs for the remainder of 2026. Any requests for modifications or
    revocation should be directed to MoAFC program staff in writing.
